Job Title: Registered Nurse Profession: Nursing Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum 2 years required License: Valid State License Certifications: BLS Must-Have: Previous Charge Experience Travel Experience Description: Coordinates utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation. Oversees the management of acute patient populations across the care continuum. Focuses on providing coordinated and integrated care to prevent unnecessary admissions or readmissions. Provides discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department. Strives to promote patient wellness and improve care outcomes. Ensures efficient utilization of health services and minimizes denials of payment among a patient population with complex health needs. Requires experience in acute hospital and long-term acute care settings. Familiarity with medical necessity, retrospective review, and concurrent review is essential. Knowledge of CMS guidelines, CPT coding, and ICD 10 coding is required. Adherence to HIPAA guidelines and understanding of HEDIS measures is crucial.
